Application for grant of permission for reprocessing of damaged fertilizers material

1.         Name and address of manufacturer /importer

2.         No & Date of authorization letter/certificate of manufacture, the name & address of the issuing notified authority/registering authority

3.        Particulars of the damaged fertilizer which is intended to be reprocessed

(i)                 Name of the fertilizers

(ii)               Quantity of fertilizer to be reprocessed

(iii)             Physical condition of fertilizer

(iv)             Date of dispatch from the factory/Port to the field godown

(v)               Location of godown where the damaged material is lying.

(vi)             Whether any sampling has been done by the Central/State Fertilizer Inspector, if so the details thereof along-with copy of the analysis report, if already received.

4.        Whether applicant possess necessary facilities for reprocessing of the material.

           If so the details thereof.

5              Location of factory where reprocessing of material is proposed.

6              If the processing facility is not available with the importer, how does the propose to reprocess the material.

7              Amount of subsidy (In case of urea/Concession (in case of decontrolled P & K fertilizer) drawn on the quality in question.

1. I/we undertakes that I/we shall comply with the requirement of sub-clause (5) of clause 23 of FCO 1985 and the terms and conditions prescribed therein for the reprocessing.

2. I/we also undertakes that I shall refund the amount of subsidy/concession so drawn from Govt. account on quality.

3.I enclose a copy of document in support of the date of dispatch of material and also a copy of the authorization letter/certificate of manufacture issued by the State Government.

NOTE: No such application for permission to reprocess the fertilizer by a

                   Government of Manufacture /importer shall be accepted by the Central

                   Government after expiry of the said period of 15 days from the date of

                  dispatch.
